In any case, I would consider a referral to a urologist. If conservative measures cannot solve the problem, a surgical procedure may be necessary. In most cases, circumcision is the procedure that is most commonly done.

You might have a mild case of phimosis, a condition where the foreskin opening is too tight/narrow to allow it to be retracted fully during an erection. You can probably remedy this yourself.

One way is to do stretching exercises; these work in the majority of cases. For one stretching exercise, take two fingers and stretch the foreskin opening as far as it'll comfortably go (like increasing its radius) by hooking a finger on either side of the opening. It may take a few tries to get right, but once you do, do this exercise for a few minutes each day for a few weeks to a month.

This usually works. If it doesn't, there are prescription creams that're supposed to help, especially when combined with stretching exercises. If after another month or so and little/no improvement (which is uncommon), you might want to consider surgical options. Note that this doesn't necessarily mean circumcision, as there are other options. Refer to the links for more info.
