If you're talking about the last episode:
It was a Julia-centric episode. Julia's flashbacks reveal that she was a brilliant doctor specializing in fertility and, unfortunately, worked for her smug ex-husband. She was offered a job with another company who were interested in hiring her on for intensive research, but declined the job in a fit of tears, saying her ex would never let her leave, and the only way she could take the job would be his getting "hit by a bus". Low and behold, he gets hit by a bus, and she's again offered a job with the mystery corporation, who sent her to live on the island. It turns out Julia is a prisoner there and needs Ben's approval to leave. We also see that she had a very ill sister who she successfully impregnated through a new method she was researching.
On the island, Jack has severed one of Ben's arteries during the surgery and refuses to repair the damage until Kate and Sawyer have safely escaped to the big island to reunite with the rest of the survivors. He argues with Julia, who eventually comes around to helping Kate and Sawyer escape if he will continue to operate on Ben. Unfortunately, Ben wakes during the surgery. He has a brief conversation with Julia (that the audience does not hear- we can only guess what was said), and agrees to let her go home if she helps. Jack removes Ben's tumor, but not before accidentally severing another artery and panicking.
Meanwhile, Kate and Sawyer are running around the island, desperately trying to escape the Others. They run into Alex, who agrees to let them use her boat if they help her rescue Karl, her boyfriend. The three set out to a cabin, where they pummel a guard and rescue a dazed Karl from a Clockwork Orange-type brainwashing chamber. As they rush to the boat, Julia stops them and demands that Alex stay while the Kate, Sawyer and Karl make their way to the other island.
Alex tearfully agrees to stay once Julia explains that "her father will never let her leave". It's then revealed that Alex is Ben's daugther- who'd have thunk it? Alex says her goodbyes, and Kate, Sawyer, and Karl make their way accross the waters.
The episode ends with Julia telling Jack that she changed her mind (about wanting Ben dead) because he's agreed to let her leave.
