Islam was a religion that spread VERY quickly in the ancient world, and generally at the expense of Christians and Polytheists. All of North Africa, Egypt, and Asia Minor were predominantly Christian, until Muslim occupation.
The crusades, although bloody and barbaric, were in reaction to centuries of hostility by various Muslim dominions. Case in point, the word "Crusader" is generally thought of in a positive light; you think of a knight in shining armor, taking up a noble cause to defend the faith. In the Muslim world, that same word is viewed with disgust.
Both faiths have made advances to better understand one another, but both still view the other with suspicion. There are also extremists on both sides that simply do not believe that peaceful coexistence is possible.

They clash when members either religion focus on differences and forget the fundamental part their faith which is peace, forgiveness and understanding.
The media does an excellent job fueling this problem. Additionally a lot of predominantly Muslim countries in the world were up until relatively recently under the occupation of predominantly Christian regimes (Palestine, Yemen, Malaysia, India, Pakistan and so on) Tensions between the Soviet Union and the United States did not help as many of these new countries became pawns during the cold war.
So it is not so much a clash between Christians and Muslims as it is a clash between Christian and Muslim extremists with everyone else caught in the crossfire.

Because one can't believe in both, and both religions claim they are the only "true" religion so you get conflict.
