You could try a commercial bleach. Try in a hidden corner first. If it works do the whole lot, then rinse off thoroughly before sealing

DON'T even attempt to replace the grout!
what a mess and you will crack tiles as you do this.
put a solution of 25 % household bleach and hot water in a spray bottle.
spray the grout with this and let it set. it will whiten the grout!
when it is clean, sponge mop the whole floor with clean hot water.
let dry and then seal the grout with grout sealer. put 2 coats of sealer on. read the directions to tell you how long to wait in between coats.
this is time consuming but well worth it to keep the grout looking good.
be careful doing the sealing. you don't want the sealer all over the tiles.
i use a small artists brush to seal grout.
look for AQUA MIX GROUT SEALER at Lowes, Home Depot or Menards.

I discovered some really great stuff to clean white grout. It's called Finazzle.
works like a charm. get it at the grocery store. on a large area I mix some with water and then use a scrub brush. or just spray it on and let it set for a few minutes, scrub it a little and wipe clean. Dropped a bottle of red wine on my kitchen floor (white grout) and used Finazzle and cleaned it right up. You
should really get some sealer from Home depot. Worth it!!! Hope it helps.

A company named Aquamix makes cleaners and colorants for tile and grout. Lowes carries it as do most tile stores. I have used it and it works great. Check out their web site http://www.aquamix.com. I do not work for them or sell their products but I have used them.

first you could try cleaning the grout with bleach and a small brush like a toothbrush lotsa work but cheap
you may have to let the bleach sit awhile let it do it's thing

You could have a professional replace and seal the grout. It'll be a hassle but worth it.
