It doesn't matter if the HR person wasn't there. Just write down your account of what happened on paper, file a formal complaint, and keep a copy for yourself. Even if the person never bothers you again, it's useful if the person ever threatens anyone else. Even if the person denies threatening you, at least it's on record.

Remember this, if you do report this person, even though you have a witness that testify the truth, you will be terminated on false accusation. The government and the Human Resources will see you as a trouble maker. Instead of fixing the problem, they get rid of you. You should get another job immediately.
The Human Resources, the Union and the government work for the boss, not for the workers.

Do not tell your supervisor/boss. If someone is harassing you, chases are the boss doesn't care or does it too. Go start to Human Resources. Actually every company should have an annonymous hotline.

As long as it isn't your boss, don't worry about it. You go your way and let them go their way. However, if the boss tells you two to work together as team members, I would gradually move laterally to another project or supervisor if the problem doesn't go away by itself given a few months time.

Tell you supervisor immediately and ask around to see of anyone else saw that person hurt you. If it happens again then fight back AT THAT MOMENT ONLY---this is considered as self defense. If you fight back like a day later, it'll look like you're the bad guy. I wish you luck and hope you don't get injured like this again, just remember to fight back but know when to draw the line.

Well I suggest you say something to your boss or take them outside and kick the crap out of them. How bad do you really want this job anyways? You could also ignore it but find a way to sabatoge their job.

There isn't much you can do without proof, witnesses, or something...this happened to me once. I reported it but lost my job for making a "false accusation" because I had no proof...just watch your back with this person or find another job where you will be safer.
