I have an LLC business in New Jersey that didn't become successful and now I need to dissolve it. I am filling out the form for it, but I can't figure it out, can anyone help me? Heres the form:

The part I don't get is number 5. Future effective date of cancellation. I read the instructions at the bottom but it doesn't make sense. What date do I write there, the date I want my business to end?

Line 5 specifically states to consult the instructions. Please look up the instructions or go to that office and request a copy of the instructions.

Update: Why did someone down-thumb me for saying "Read the instructions"? I don't live in New Jersey, but tax forms are tax forms. If it says, "see instructions", then.....duh???

"Check the instructions" for line 5 makes much better since than the bad advice given my people who just "guess" about it.

It wants to know the date you'll actually be out of business, if that date is not the same day you're filling out the form.

Let's say you're planning on closing up shop next Friday. That's the date that goes in #5.

Caution: you can't put in LAST Friday as your closing date - since the form says it can't be before you fill out the form. You also can't put down six or seven Fridays down the line, since the form says you have to name a date that's no more than 30 days after filling out the form.

If you filed say Monday (Feb 12) the date you want the cancellation of your business to take effect could be anytime after the 12th of Feb up to 30 days after that date. In other words you have to put a date to cancel your business which falls within the date you send/drop off this form and the cut off 30th day.

Arbitrarily, if you filed Monday Feb 12 and requested the cancellation date be the 2nd of Mar you would be within the 30 day limit they allow. If you requested the cancellation date as Mar 31 you would be beyond their 30 day limit which they do not allow and which would probably void your request in its entirety until it was corrected.
