First off, it's spelled Evanescence.
Secondly, you'd have to ask the songwriters themselves to get a precise answer, and that would miss the point. Music is art, after all, and nobody's going to get the same thing out of it. Its beauty and meaning are in the ear of the listener--the song will mean different things to different people.
My take on it: the writers are trying to capture the uncertainty of depression, where a person knows something is wrong but isn't sure how to go about fixing it ("I can't go with me/Wonder what's wrong with me"). Lithium is a commonly-prescribed anti-depressant, thus the title. The ending lyric--"I'm gonna let it go"--seems to indicate a resolve by the protagonist to fix whatever's wrong.
One might think this material is too heavy for someone so young (Amy is 25), but I say it shows a refreshing maturity. Just in time, too: all the Christinas, Britneys and Hilaries on the radio airwaves were giving my ears cavities with meaningless, sugary songs. Growing up in the 80s, I always thought girls were hottest when they rocked. I'm glad Amy thought so, too.

Choosing between happiness and sorrow. Lithium is a metaphor in the song. Amy said she never used the drug before but it fit the song. Here, here is a link to explanations to all and every single one of thier songs ever released. So it's not about the element Lithium in the periodic table (proton count 3, atomic mass 7), neither is it about the drug that they use to treat manic depression.

I don't really know the song, but lithium is a rather poisonous metallic element that was used for depression and bipolar disorder until they found better stuff. That's where Nirvana's song comes from

Lithium is a mood stabilizer, most often prescribed to patients who are bi-polar. Bi-polar patients experience feelings of extreme highs and then feelings of extreme lows.
In the song, Amy talks about wanting to stay in love with her sorrow and then at the end she says she is going to let her sorrow go. The song is about wanting to hold onto the sadness but at the same time wanting to experience happiness, but she's afraid of it because it is somewhat new and unknown.
