Well the game developers release the game cheats to the online media, they place them on their sites and all so no one actually is the 'first' to get the cheat codes, they are there created by the developers and placed on their game website and released to the online media for free use.

I am not sure how they are put there, but i am almost positive that they are purposely(with the occasional accident) placed bugs designed to make the game more fun or less challenging. As for the first people to know, i would probably say the main design staff would probably be the first.

I bet it is the programers. The probably do it because everybody gets totally pumped when they find cheats and easter eggs. I heard in the ancient days, they used to make them so that they could rage through the game fast when they were searching out bugs.
