Truthfully the best way to thank your boss for your raise is not by a thank you card but by continuing to work hard and do a good job and improve your quality of work . He or she gave you the raise since you obviously are so far doing an exceptional job and they are rewarding you for that and giveing you an incentive to keep up the good work. You earned the raise so sending the boss a thank you card for a raise you have earned. Thats just me though and everyone is different and if you feel better sending a thank you card then do it . And IF I were to write a thank you card for something like this It would say . thank you for the opportunity to work and grow within your company. I hope that I can continue to improve and grow with your company.

Dun send a card. You earn the increment with your hard work. Instead focus on doing a job well and prove to your boss that he had indeed make a wise decision.
