A lot of tattoo parlors have professional piercers. They use a needle and a small cork to catch the needle until they put the stud in. It will make you eyes water a little. It only hurts a little bit.

They take a pretty large gauge needle, and stick it thru your nostil. It hurts like heck. They don't use clamps or anything, it's completely free hand.
