Form Handling Codes?

Question

I have a login page...
when an employee logs in, the login username and password should be sent to an email id which will redirect to appropriate log base.and the employee should be redirected to another url.

there are only 3 fields:
uname = textbox
paswd = passwd textbox
submit button

kindly give me a full code.
in HTML or asp or php
i dont need asp.net or cgi scripts
