A judge cant ban someone from a church unless its part of a restraining order or something.....the pastor can fille trespassing charges but I dont know of one who ever has

If you are entering into the exclusion area specified in a restraining order (e.g. 200 yards away from a particular person) then yes, that would be a breach of the order.

Not in the United States. However, if you are disruptive in a church you can be ordered to refrain from doing that.
