Certainly, there are multiple causes for such a phenomena. I can mention a few that come to mind.
I suspect that this would at least some of the time have something to do with experience. Some people apparently experience more cloistered, protected lives than others. Some, perhaps due to a charming appearance, charismatic persona, or surplus wealth, are able to glide through their days with relatively little anxiety.
Of course, you may be speaking about someone who is poor, unfortunate, etc. and yet does not recognize their sufferable condition. Perhaps they simply are not attached to the values society has endeavored so hard to instill upon them.
Whether or not it is possible to have fire without smoke (action without negative effects), it seems to me a laudable effort to endeavor not to impose one's being upon others. I myself live according to this principle, and while I occasionally step on some toes, I try to avoid it as much as possible. Thus, I practice the Vedic principle, ahimsa (non-violence). I have found it possible, if only by living simply. It was Gandhi who said, "live simply so that other may simply live." This is a noble thought, and worth pursuing.
Other modes of behavior are possible, such as passionate or ignorant action, but these are inferior in terms of the happiness they provide, and therefore ought to be avoided. Detachment from worldly activities gives one peace of mind. Man was born to nature, and only later creates the burden of self-imposed labor, while pursuing manufactured duties.

Most people live dreary hard-working lives. Look more closely at the way people spend MOST of their time. Sure, you'll notice some people prancing through life not caring about the consequences of their actions, but check out rush hour in any city or town and you'll see most people are working to survive. As for the fantasy worlds we inhabit, well, it's great we have an imagination to drag us occasionally out of that dreary daily routine. Many young people are still being supported by parents, and those kids really don't seem to have any cares or responsibilities; but their turn will come.

We have the same confusion, really...but have we really thought of one thing, like...what really is reality?
I guess that is the problem...until now we still are not so sure (and there hasn't been a concrete answer on this, either) what reality is. We believe in one reality, that is because we have been taught of that since we were children, and those who taught us that reality were also exposed to that reality since they were children...did you somehow get my drift? We wouldn't even know that our reality is right or wrong for others if we never knew of the other outside worlds...right? Like for example, have we ever really understood the practice of harakiri in Japan? That is one of their strongest realities...
Time (generation), culture, the environment...these are just some of the factors that set us apart from the others, that is why we believe in different realities. That is why sometimes--if not often--the views of the people extremely clash, even if we all live in one planet.
Perhaps it's really not that some people are so out of touch with reality...but maybe 'your' own reality, because if you really come to think of it, 'you' too will be out of touch with other's realities, mind you.
So I guess what we also need is a little in-depth view of things: try to understand why people act the way they are--them being so out of touch--and tell them what you feel, too. There is no better way of getting to a consensus other than communication. Perhaps, if we try to get into their shoes, too--and them getting into ours--people wouldn't seem to be so out of touch with reality.
But that doesn't mean they have the excuse of doing things that are already wrong, different cultures or not. Especially negative acts with obviously negative intentions. Murder is one concrete example.

jkk k is a nut. but my answer is simple. its an escape from reality using alot of gods word as an excuse simply because there to darn lazy to get in the picture and realize we cannot live in the truth and it really is not expected of us to do so.
