Revolution X for the Super Nintendo was pretty bad. The arcade game was fun, but by the time they scaled the graphics down for the Super NES, you couldn't tell what was going on at all. All of the enemies were identical, the 16-bit renditions of Aerosmith music were ridiculous, the "action" was frequently indecipherable and repetitive, and it froze up at least once an hour.

By far, though, the worst game that I've ever played is The Blues Brothers for the Super Nintendo. I really honestly think that that game started life as something completely different, then Atlus got the rights to make a Blues Brothers game and said, "Hey! We're making a game already; let's just put the Blues Brothers in that!" The game starts out in a forest, for no apparent reason, and has you jumping around and avoiding squirrels, living lawn mowers, and bear traps. For some reason that is not readily apparent to me, you find cupcakes along the way. If you eat the cupcakes, you get really buff and you burst out of your suit jacket and you can jump higher.
Then you go underground to what is possibly a sewer, possibly an electric plant. I like to think it's a sewer because part of the movie took place in a sewer, but I really don't think that's what it is. At any rate, the place is filled with arcing electrical beams that kill you instantly. You've got to climb up and down ladders, and if your finger ever-so-slightly taps left or right on the control pad, you immediately plummet off of the ladder and into your certain, immediate death at the hands of the random electricity.
Assuming you manage to survive that, you get taken to a level where you ride a dragon. No clue why. I would say that it's, like, a Spirit of the Blues, or something, like a Blues Dragon, but it's gray. Also, it doesn't seem to sing or anything. So, the Blues Dragon is constantly floating up, so you've got to constantly press down (again, ever-so-gently) to keep it steady and on-course, because if you touch a ceiling, wall, or floor at least once, you fall to your doom. Also, you might touch one of those arcing electric bolts and not even fall to your doom, you just sort of instantly die.
Assuming that you survive that, you go to what looks like a jailyard with a fence in the background and - get this - another fence in the foreground. The fence completely obscures your field of vision, and when you walk, the foreground fence scrolls at a different speed than the background fence, so it creates this weird visual effect that can probably cause vertigo and possibly seizures in like 1/2 of the population.
I don't remember how many stages this game has; I only remember thinking that it would NEVER BE OVER, and when it finally was, I got a still image of the two of them propped up against a jukebox (I think it was the title screen, actually) with the word "Congratulations!" across the top of it, and some credits. Quite possibly the least satisfying ending in video game history.
On the plus side, it did give me a story to tell, so I guess it wasn't all bad.

To respond to the first answer given, Duck Hunt was and is still a good game. Anyway, a really bad game is any Final Fantasies. I just offended many people, i'm sorry, just not a fan of RPGs and anime.

The worst game in the IGN's opinion is a paintball game called Extreme PaintBrawl. http://pc.ign.com/articles/160/160455p1.html

It got the lowest score in IGN's history. Ha, pretty sorry for the developers =)

Bayblade (?spelling) for the PS2 - so hideous, insipid and truly, truly awful even my five year old asked for special allowance to say 'a bad word' to describe it to his Granny.
