How do I start, do I go to the 4 year, the graduate med school, I want to be a doctor... what do I need to do- what is the plan?

Get your bachelors. Doesn't matter in what. Just make sure you take all the prerequisite courses required by med schools.

If you want to go to John Hopkins Medical, you better have top notch grades and MCAT score along with some impressive extra curricular activities. Your undergrad doesn't matter much.

Why do you want to go to John Hopkins so badly? To be honest, it's so tough getting in to med school, it's quite an accomplishment to be accepted anywhere in the US.

You need to go to college and take basic courses that you need to support the courses you take in medical school. The school will tell you what to take. Then you need to take the medical entrance exam. Then you need to be better than about 80% of the people who apply.
After you get through medical school, you have your M.D., but you must do a year of internship, working in a program under other doctors and then take the state licensing exam.
Then if you want to do anything serious, you need to take a residency, which may last from 2 to 6 years depending on the specialty, during which you learn the details of a particular area of medicine.
