If not - about even with the tops of your inner shirt pocket.
(Basically, wherever you need to put it so it will look good!)

Lapels by custom on the left lapel. Some lapels are also tie tacks too. This is from military custom of putting ribbons on left side of breast pocket.

On some suit coats, there's a little cheater button hole on the lapel. It goes there.

Absolutely not. Just throw a dozen or two I support the troops magnetic signs (made in china) on your car, that should do the trick.
