They found out that they had the wrong babies and they switched. Isaac, who Hope and Bo called Zack, died when Chelsea hit him with Bo's car. He donated his liver to Claire and his corneas to Abe. JT was living with Lexie and Celeste. Now that Lexie is missing, who knows.

Zach was run over with a car by Chelsea and died. Abe has JT.
