No. The Department of Labor is not your mother. They only do the basics. They make sure you get paid as agreed, and that you don't work in a sweat shop. That's it.

It is the law that you have to receive a break. There are no laws mandating what you do on your break, or where you take it.

I believe it depends on the amount of people working there.Check with the Department Of Labor in your city.

Don't know. Are you covered by any labor agreement, (UNION). If so it would be stated in that agreement. Ask your shop steward.
