I think the best way to let her know you care is to show her, not just tell her. Maybe you could make her a mix cd of songs you guys both love or that remind you of her. That might remind her that she likes who she is when she's with you, and you might save her from becoming another person. The message you want to convey isn't about her change, its about how much you value her friendship the way it is now.
