Medicare is federally funded insurance for citizens over the age of 65, or disabled. It is not based on income. Well, okay, in 2007, your Part B (doctor bill) coverage premiums are based on income.

Medicaid is state-funded, and thus the qualifications vary from state to state. If you have no income, you would likely be covered. Google "medicaid" and your state name.

I'm not sure what an HRA is, unless you are talking about a Health Reimbursement Arrangement. This is the account you can get if you have a HDHP and do not qualify for a HSA.

If you are unemployed, have a very low income and have no assets you can apply for Medicaid Managed Care.
