I have a form to fill in that needs my qualifications (letters) added after my name.

It would depend on the purpose the information is required to be given.
If the form is an application for a teaching position, you can list your quailfications in the order in which you got them, adding the subjects in brackets, such as B.A. (Economics), Ph.D. (Industrial Relations), M.B.A. (Finance and Marketing). You can also add the name of the University/Institute for each degree.
If the form is for a job unrelated to teaching, I would put the highest qualification obtained. In the above example, I would not include B.A. unless the position applied for calls for some exposure to economics.
If it is for a survey or for other purposes, I would only list the highest degree obtained, followed by other degrees unrelated to the earlier one, such as Ph.D., M.B.A.

If you are claiming a doctoral degree do NOT put it as Dr. Jim Jones BA MA PhD. Rather Jim Jones BA MA PhD, or Dr. Jim Jones.
