I need to know how to get the Key Index off a SIM card?

You can't. If you could the SIM card could be cloned and you could use other people's accounts. This would break GSM security wide open. GSM security is completely dependent on the security of the Ki.
Even the phone can't read the Ki. The network gives the phone a challenge key (random number) and the phone asks the computer on the SIM card to run an encryption algorithm on the Ki. The SIM (via the phone) then gives the result back to the network which has performed the same algorithm in the Authentication Centre (AuC) This is the only other place that the Ki is kept
