the park definetly. You wont be able to talk in the movies. If you go to the club, someone may endup dancing with your date, An amusement park is just to much going on for a first date. And everybody knows you shouldnt rent a hotel room until the second date. hahahaha (just playin') the third date......... But really, you can have a little picnic and walk and talk and bond more if you go to the park.
P.S. if you two dont do much talking; go to the movies. hahaha

5. LoL... no.
1. Eating is good, but you can't get to know someone staring at a big screen in a dark room with a bunch of other people, unless the movie is REALLY lame AND you decide to stay around...
2. not bad, clubs can be a bit roudy & can contain annoying drunk people
4. not bad either, maybe an option for later :-)
3 Definitely. Not only do you get to strolling around a park talking you get activities, food, fun, shared experience, laughing, adrenaline. Go for it!

Either 4, 3 or 1 depending on your personalities. Definitely not 2 or 5 .
You need a date that allows for easy conversation opportunities so you get to know each other in order to find out whether you want a second date!

4, then possibly 5, if things went well. It's difficult and somewhat uncomfortable to talk while eating and really difficult during a movie.Same with a dance club, too noisy to really 'chat' and get to know one another, though drinks somewhere low key would be cool. An amusement park is kind of 'Grand' for a first date, if things aren't going well one might feel stuck.

Ask your date what they are into. If they don't drink - don't go to a bar. If they don't like dancing - don't go to a club. Knowing likes and dislikes can definately help in the decision process, especially for a first date.
Oh, and you generally want to be talking on a first date in order to get to know the person. So make it a point to at least have an hour of talk-time, and then you can go to a movie or club.
Just do what's fun for the both of you. Commonality, baby!
