It's nice to know I'm not the only one who gets confused about this kinda stuff. HAHA.
And to those who say "Choose ME as your best answer..." well, I'm not because you're just answering for points.

Leave Jacob out of it when you're saying whatever you're trying and it's easy. My anniversary is correct and still correct when you add Jacob back in. As in Jacob's and my anniversary. This works in any situation where me, my, I is combined with another person. Use the singular to get the right form for yourself and then add the second person.

Jacob and my anniversary....alway take away the 2nd person and see if if sounds right, for example, take away "Jacob" and "my anniversary" still makes sense

When the sentence would sound right with just one person (you) involved, the way it sounds then is the right way to say it.
It is my anniversary. (good)
It is I's anniversary. (stupid)
It is Jacob's and my anniversary.
By the way, Happy Anniversary!

Jacob and my... it wouldn't be I because you wouldnt say "i first anniversary" you would say "my first anniversary" and the other person always comes first... so Jacob and my first anniversary

It would be Jacob's and my first aniversery.
Look at it this way does it make sense when you leave jacobs name out.
For instance it doenst sound right when you say "I first aniversery" or Me first anv." right but "my first aniv.: Does
Just do the same when you do it with anything else similar to this but remember you are always the last person to be announced. LIke jacob and i are going out to eat.
