I will close my eyes, take a deep breath, and slowly let it out. Then i will take a long look that the task at hand and "plan my attack." I like to picture that it is only me and the task at hand (this task may be talking to a person, listening to a lecture, reading an article, etc.)
If i feel myself losing focus, i will gently bring myself back to the task. Not forcefully, persay, but gently. If i just can't seem to focus, i'll usually take a short break (5 min. max) and then come back refreshed.

Honestly, I don't know. I just do it. I have always been able to focus on the task at hand and can handle multiple tasks without losing focus. I'm also driven to get something done and do it well, that helps me stay focused...motivation, I guess.

Relaxing and not being afraid or nervous. Do whatever you can to achieve being calm.
Meditation
Warm drink - herbal tea, water, milk
Quiet atmosphere - room with door closed
if not Soothing music - plug your earbuds into a song that doesn't have words in it and has a slow beat.
Sitting or lying in a comfortable position.
Having a good motivation for what you are doing.
In life - all the above. Read books on the subject.

Exercise regularly. Eat right. Good sleep. Getting up at the same time each day. Have a spiritual connection. Follow through with your goals.

Ritual changes your state of consciousness, so that the mundane, or everyday challenges fade, and you can then see what you're looking for more clearly.
