Easy question:
When Derek cam home one night from work he found his wife, Addison, in bed with his best friend, Mark, and left them there. Derek wound up in Seattle where Richard hired him. Meredith wound up in Seattle to start her internship and watch over her mother. After the intern mixer Meredith and Derek wound up sleeping together. She does that when she is drunk. They formed a relationship and then Addison showed up to throw a monkey wrench in the works. Derek tried to see if there was anything left in his mariiage but that did not work. Mark arrived to try and bring Addison back to New York. Mean while Finn the vet tried to form a relationship with Meredith. The reconciliation failed. Derek and Addison Divorced. Meredith had an appendicitis and made a choice between Finn and Derek and chose Derek.
They started back slow but wound up speeding things up and then slowing them down a tad to where they are now.

The marriage finally finished breaking up when McDreamy & Meredith did the deed after being apart for months. Derek realized he loved Meredith, not his wife.

they originally met at that bar across from the hospital..he went to seattle to get away from the wife who cheated on him
