St. Peter's Basilica in the Vatican. When there's no official service there, anybody can come in, right up to the santuary and way down to St. Peter's crypt below.

When I was there the time before last, I ran into a small wedding party from the US (Wisconsin) consisting of the bride and groom, the best man and the maid of honor, two sets of parents and their own minister (not a Chatholic priest but a Protestant minister). They conducted their ceremony up in the altar (the Cathedra Petri) while the most of the other tourists went about their tours not really paying that much attention to the wedding. In other words, the basilica is so large, the wedding ceremony did not disturb the regular tourists and vise versa.

I talked to the wedding party after their ceremony and they told me they had checked with the Vatican way ahead of time and were told they did not need a special permit if their group is fewer than a certain number (I think it was 20) and that they would not do anything to disturb the other tourists (loud music & other noises or impeding the flow of people in the basilica).

Since you most likely will not have more than a few people, you may want to have your renewing of vows in the basilica. To make sure you're in the clear and have someone make the arrangement for a minister/priest there for you, check out this link: http://www.weddingsitaly.com/htm/churches.htm
You can have your ceremony in other venues of course, as the website indicates.

Congratulations, best wishes and good luck tot he two of you. God bless!

No it's not legal in Italy. And I'm guessing since they house Vatican City that it will probably be a long time before it is. Also just on a sidenote the mafia originated in Sicily (where my family is from) not Italy. They are different countries.
