Not great to very bad.

The kidneys are very, very important organs, they filter the blood and keep it clean. Without them the body can not get rid of extra water salt, and everything else that goes out in urine.

If your Guinea pig has kidney disease, but they are still functioning, even if its at 15%, you can probably manage it with diet so that your little guy/ girl will be around and happy for a while.

However, if the Kidneys are shutting down (renal failure), there isn't a lot of time left before wastes start building up and the animal dies. In humans, they can filter the blood through a dialysis machine, which can keep you healthy for years until you get a kidney transplant. In Guinea pigs, it is unlikely that you would be able to perform dialysis, afford it, or be able to even find equipment suited to guinea pigs. That is assuming you are willing to put you pet through dialysis 3-4 times a week for the rest of its life. (personally, I would put the animal down at that point due to quality of life concerns)

The good news is that your vet seems to think you got it early in time to manage it and let your pig have a good quality of life for a while.

as bad as it is for anything. kidneys are vital organs (the important word being "vital"). see a vet.
