Where can I find a map--a VISUAL MAP--State by state with counties outlined, Metropolitan areas outlined around the counties, with a list of cities in each

Try the Census Bureau:

Your best bet would be Google Earth, as this will show you everything you want to know... still it would take some time to get all that information and wont have a simple list of cities for each county.

I'll give you a link to a site I found that shows county maps state by state - it shows their outline, name, and the county seat of each county. As for city outlines and a list of cities... again, you're out of luck.

Find yourself a copy of ArcGIS, download census data (it's free), make your own map and print it out on a plotter.
