It is the Jack of diamonds. And the option is that you can play with the jack of diamonds being worth negative ten points (a very good card to get). It makes the game a little more interesting because people can no longer simply play to not win any hands (or shouldnt if they want to win the jack). Hope that helps :)

If you choose the option, the jack of diamonds becomes worth negative 10 points. This affects the game like how to penalty for the queen of spades affects the value for the ace and king of spades. This is, in a game with that option, people tend not to pass the diamond court cards and the person with the diamond jeck would tend to keep a longer diamonds.

The Jack of Hearts or Jack of Spades are known as "one-eyed Jacks" (because the face is in profile - you can only see one eye). They are wild cards in Hearts and Poker (and a few other, more obscure, card games). You can substitute a one-eyed jack for any other card in the deck, for example, in a game of draw poker. Say you've got a 2-3-4-5 and a 10 in different suits, you can substitute the 10 card with a one-eyed jack and have a straight.

Note that whenever I have done myself or seen it done by other people in poker, you always say "one-eyed jack" when you show your cards. I don't know if this is custom or necessary.
