First, you need to have a copy of your return as you had originally filed it. Copy the numbers from that return into the column labelled "as originally filed" on Form 1040X. Then you prepare a new 1040 with all of the numbers from your original return, except you include the things you needed to amend. This return is used to fill out the numbers on the 1040X in the column "as amended" or "as adjusted". The differences between the two columns are entered into column 3. Fill out the rest of the form to determine what additional tax you owe, or what additional refund you are due. On page 2 of the 1040X, you need to provide the reason why you are filing an amended return. Mail the 1040X, 1040 as originally filed (with "as originally filed" labeled on the top of each page) and finally, the 1040 as amended (with "as amended" labeled on the top of each page). Send it in, along with a check for the tax due, if any, to the IRS.

Go to www.irs.gov and print out the form 1040X in the Forms and Publications list. This is what you have to use to file an amended return.

IRS website - for 1040X shows the changes. Attach a corrected return to the 1040X and a copy of the original return.

Don't forget to amend the state also.
