I ran it passed Microsoft Word and no wiggly lines appeared, so I guess the answer is 'no'!

A comma is suggested after car, to make the sentence clearer.
As I approached my car, the alarm went off.

No, it does not. An example of one that did need one would be:
Suddenly, as I approached my car the alarm went off.

I think a comma is optional here. It seems to be appropriate. The phrase "As I approached my car" is slightly misplaced and awkward. That phrase is better placed at the end of the sentence.

I would say the comma would come after the word car,but This depends on how you read the sentence for instance if you read it fast it wouldn't need it whereas if you read it slow it does need it,and it also depends if you have an asthmatic condition.

As I(noun) approached(verb) my car, the alarm(noun) went off(verb). Otherwise it would be a run on sentence.

I guess it can work either in this short sentence but if you try saying it aloud, you would have a natural short pause after car.

The sentence itself is the best judge. Ask the sentence if it needs one!
