Is this thru the DVM, SS office, or can you print a form off line? I'm not sure where to start...

You have to change it with the Social Security office to make it legal. I believe there is a form online that you can fill out. After getting married I had to fill out this form and make a visit to the Social Security office to make the change. Give you local Social Security a call and see what documentation you need to take and make an appointment.

If this is not a change through marriage, you start by legally changing your name. Any local attorney can give you all the details. Then, once the name is legally changed, you go to DMV, Social Security, Voter Registration, credit cards, any where you have your name recorded, and send them a copy of the order changing your name.

You have to go to all of those places...DMV, Social security, The banks, and notify all of your creditors. It takes forever and if it is due to a marriage you need certified copies of your marriage certificate. You may need to go into DMV and Social Secutiry to do this. it all depends on what state you are in. Good Luck

Depends,If you got married you take marriage lic to Soc sec. office , then to drivers lic to change name .If you want to change name just because, you go to court house, there is a fee,and it takes a little while to be approved, they can deny for certain reasons.
