The first churches were actually in people's homes. Christianity was outlawed in the Roman Empire until the 4th century when it was made the state religion by Emperor Constantine. When they started to build churches they used the Roman basilica as their model. The Roman basilicas were law courts that were open to the sky. So the first churches were basilicas that were roofed. They had two rows of columns down the sides. So there were three aisles. Later churches were built in the form of Greek and Roman crosses.

The early chirsitan churches were organized in nearly the exact same way as the modern Catholic and Eastern Orthodox churches. The only difference is that these communities were forced to meet in secret. When Christianity was legalized, the pre-existing church structures became publically notable and gained secular power inside the communities where they were.

By structure do you mean how were they run? Or do you mean architecturally speaking?

Acts chapter 2:42-47 outlines how the early church functioned:

"All the believers devoted themselves to the apostles’ teaching, and to fellowship, and to sharing in meals (including the Lord’s Supper), and to prayer.

"A deep sense of awe came over them all, and the apostles performed many miraculous signs and wonders. And all the believers met together in one place and shared everything they had. They sold their property and possessions and shared the money with those in need. They worshiped together at the Temple each day, met in homes for the Lord’s Supper, and shared their meals with great joy and generosity —all the while praising God and enjoying the goodwill of all the people. And each day the Lord added to their fellowship those who were being saved."

They were under the direct leadership of the 12 apostles.
