He died painfully. There was an incident at the Over the Edge pay per view i think. Here is some proper info:

Death
Hart (as the Blue Blazer) fell to his death in Kansas City, Missouri, on May 23, 1999, during the WWF Over the Edge pay-per-view event; he was in the process of being lowered into the ring from the rafters of Kemper Arena for a booked WWF Intercontinental Championship win over The Godfather. In keeping with the Blazer's new 'buffoonish superhero' character, Owen was to be lowered to just above ring level, at which time he would release himself from the safety harness and then comically fall flat on his face. He had performed the stunt only a few times before, and was worried about performing the stunt at Kemper Arena due to the height involved (Owen had a fear of heights).

Owen performed a practice stunt earlier in the day, and the stunt went forward on the show as scheduled. His wife Martha has suggested that, by moving around to get comfortable with both the harness and his cape on, Owen unintentionally triggered an early release and fell 78 feet (24 m) into the ring, smashing his chest on a padded, but still tough, ring turnbuckle.

TV viewers at home did not see the incident or its aftermath, as the WWF was transmitting a promotional video package for the match, and only showed the audience while Owen was being worked on by medical personnel inside the ring while WWF television announcer Jim Ross repeatedly told those watching live on pay-per-view that what just transpired was not a wrestling angle or storyline and that Hart was hurt badly.

Owen was transported to the Truman Medical Center in Kansas City, where he was pronounced dead on arrival. The cause was later revealed to be internal bleeding from blunt chest trauma. The WWF chose to continue the event, which drew a great deal of criticism over the following weeks, especially as the main event of the evening involved The Undertaker (also known as the "Deadman") winning the WWF Title.

I also know a website which shows pictures of him falling to his death.

It's disrespectful for me to show it but that's evidence.

Have a nice day. And you are very much welcome!

ok this is what happened. from what i heard... owen was given a storyline that involved him dating a woman. owen wasnt comfortable with this because his kids were watching the show and he didnt want to confuse them so he passed up the storyline which isnt a smart thing to do considering he was lucky they came to him with anything at all. vince mcmahon wasnt happy about this so he brought owen hart back as his old gimmick called the blue blazer. it involved him wearing a stupid costume and a mask to cover his face and also a cape on his back. when over the edge ppv came along he wanted owen hart to descend from the rafters up above like Sting use to do in WCW instead owen was suppose to come down half way and then release his harnest makin him fall to the ring just to get everyone to laugh at him though as we all know that didnt happen. very few people really know what happened up there but supposingly he fell ALL THE WAY from the top and his head was the first thing to hit the turnbuckle and he was pretty much dead then. i was watching the ppv. we didnt even know he was up there and next thing u know ( u dont see it ) but the camera's looked away from the ring and J.R. is studdering. he apologizes and i believe they went to a promo. when they came back J.R. told everyone wat happened and that Owen was dead. The show went on but many of the wrestlers broke out of character and when it was their time to do an interview before their match some started crying for example i remember Jeff Jarret and Debra crying. i was a young teenager then and very very shocked. ill never forget that night.

okay what really happened was Owen Hart had a popular second character, The Blue Blazer, and at Over The Edge in Your House 99 he was going to be appearing as the blue blazer, he was being lowered down to the ring on cords and he had a clip on the restraint, so he could let himself down once he reached the ground, unfortunately his cape got tangled into the clip and it unsnapped the restraint and he fell (not quite sure how far) to his death, he hit the turnbuckle, and broke his neck, he died in the hospital later that night, the Hart Family owns all footage of that PPV and even if they didn't, nobody was expecting this to occur so their were no cameras pointing towards him when it happened.

In a pay per view event called "Over The Edge", Owen was going to wrestle as his "Blue Blazer" character. An overzealous super hero who catch phrase was similar to Hulk Hogan. "Remember to keep training, saying your prayers and drink your milk, kids!!! Wooooo!!"

His enterance to the ring for this pay per view was to "fly" into the ring from above. He was straped to a pulley system in which was supposed to lower him slowly to the ring directly under him. Unfortuately the pully system gave away and Owen dropped a good 25 to 30 feet. his head connected with the top turnbuckle and that caused him to snap his head back to the point of breaking it and causing instant death.

The actual death was not shown on pay per view as they had a few second delay. But Jim Ross was describing the event as it happened. But not to the detail that I have just described. I found out the details from friends who went to the pay per view , live.

He Pass Away in May 1999. Stand on 80 feet then his rope broke or rip, he fell 78 feet in about less than a minture.

He was being repelled down from the rafters for his entrance as the Blue Blazer to the match, and something went wrong and he fell to his death. I think he may have smashed his head on a turnbuckle when he went down. Not sure on that though.
