You need to provide more information. Question 2 doesn't even make sense, and Question 1 can have many formulae that pass through those points:

straight line: slope = 12, y intercept = 3, so the formula y = 12x + 3.
Binomial: x^2 + 11x + 3
Trinomial: x^3 + 6x^2 + 6x + 3
... etc.
