What things do I need to worry about in my car to keep it running when the temperature is really cold outside?

Good battery, fresh gas with/gas treatment, proper antifreeze, proper oil, proper tires.

Use an antifreeze tester to make sure your antifreeze is good for the expected temperature in your area, make sure your engiine oil is fresh, likewise with power steering fluid, trans fluid, etc. Aged oils tend to lose their additives and thicken up more in the cold.
If it gets cold enough, you may want to get a crankcase heater. Easiest kind it a dipstick heater, this is basically a plug-in oil dipstick which heats up and keeps your engine oil warm. To save energy, this can be put on a timer so it turns on a couple of hours before you normally use the car.
Make really sure you have an emergency blanket and other gear in the car in case it breaks down somewhere. Make sure you have the antifreeze windshield washer solvent in the tank, and keep it topped up.
Sounds like a lot of stuff, but it sure gets cold walking along the road otherwise.

Keep all your fluid levels up, and run the car often, to keep the battery in good shape.

Keep at least half tank of gas, and a thing of dry gas in it every so often. keep antifreeze level, good tires,and a good battery.....

Never let your gas get below a quarter tank. It will freeze up the lines if there's hardly any gas. Not good!

Make sure your battery has enough charge or isn't to old. Also flush your radiator every couple years.
