After they type their answers on the downloaded pdf from from a website, they can send this via email back to the person who's looking for the form.

How do make the PDF file so this will happen?

Why not just use HTML and create the form on a webpage, and have the form information sent to you via email using a CGI formmail program script, once they click on the submit button?

Other than that, I believe that you would need to have a full version of the Adobe Acrobat program, have your visitors download the PDF file of your form, fill it out, then save it and resend it back to you. Seems like an awful lot of work to expect your website visitors to do.

If you can't do it with a PDF file, why not just create your questions in a word document file, allow them to download it, fill it out, and send it back. Seems easier (and cheaper) than using a PDF.

You have to have the full version of acrobat.

I've heard of people being able to take a Word dot file and convert it to pdf like that, but I don't have any experience with that.
