There are no hard and fast rules. on a first date there is nervousness and some expectation. By the time of the second date this will have been assuaged somewhat.

Why not! Love at first sight is possible but not the only method of starting a relationship. Quite often you are forced to work with someone you instantly dislike but they can end up your best friend. People change, opinions change, everything changes so why would the second date need to be a rerun of the first.

anything can happen.. first time or eighth... the past is past... live in the now and be happy that things are going great... maybe the time in between one or both of you learned a life lesson that has made your lives together easier and more enjoyable
