each European countries will have the UEFA Champions' League & UEFA Cup places allocated to them each season. usually the teams will qualify via the league position at the end of the season & also if the team wins their domestic cup competiions.
currently in ENGLAND, the top 4 teams in the Premiership will qualify for the UEFA Champions' League. the Premiership champions & runners-up will progress directly to the group stage while the 3rd & 4th place teams will enter the competition in the qualifying rounds.
the UEFA Cup places will goes to the winners of the domestic cup competitions, the League Cup & the FA Cup. if the teams wins the cup & also finish in the top 4 of the Premiership standings at the end of the season, then the UEFA Cup will goes to the 5th & 6th place teams in the Premiership.
the FA Cup is open to ALL professional football teams in ENGLAND of all ranks, from the Premiership right down to the non-league teams. the competition is basically a straight knock-out match played once or twice if the first match ended in a draw. at each round, teams will be drawn to face each other, no seedings as this is an open draw, so a potential upsets is always on the cards.
the lower league teams will enter the competition in the first round while the Premiership & League Championship teams will enter the competiton in the 3rd round. the final two teams left in the competition will play in a neutral venue (Millennium Stadium in Cardiff, Wales at the moment) to decide the FA Cup champions.

Taking the example of the English Premiership:
1st and 2nd in the league qualifies for the Champions League group stage automatically.
3rd and 4th in the league qualifies for the third qualifying round of the Champions League.
5th qualifies for the UEFA Cup 1st round.
League Cup winners qualify for the UEFA Cup first round. If the winners has already qualified for European competition via the league (that is, finished in the top 5) then the spot goes to the sixth placed team in the league. The runners-up in the League Cup DO NOT qualify for the UEFA Cup.
FA Cup winners qualify for the UEFA Cup first round. If the winners have already qualified for Europe then the spot goes to the runners-up. If the runners-up has also qualified for Europe then the spot goes to the seventh placed team in the league.
Higehst placed league team that has not otherwise qualified for Europe (could be 6th, 7th or 8th, depending on the various situations above) goes into the Intertoto Cup. However, this is by invitation, and a club can decline. If they do so then the spot goes to another league (meaning another league has two Intertoto entrants).

For UEFA Champions League, the teams who come in the top four positions in their respective Eourepean Leagues, qualify .
In the UEFA Champions League . The top two teams qualify for group stages but 3rd and 4th places have to go through qualification rounds.
The 5th and 6th place teams in their respective European LEagues qualify for the UEFA Cup.
For FA Cup all English teams from all divisions play and do not need qualification .

Bryan answered a bit too national I think. There's also countries like Luxemburg or Malta where even they champions needs to qualify for the Champions league and even the 2nd needs to qualify for the UEFA-Cup. It depends on the 5-year-ranking of the UEFA. Based on that every country can send 1 to 4 teams. Especially for the UEFA-Cup there's plenty of ways in each country to spread them like qualifying through the league, through the cup(s) etc.
And there's also the chance to qualify by fair play.

Depends on their last standings on theirleague.
For premiereship, 1st and 2nd automatically play on group stage of the Champions League; 3rd and 4th have to go through at least one qualification to be able to play in group stage.
5th and 6th paly for the UEFA Cup, 7th and 8th to the Inter Toto Cup, and the rest is dancing in the moonlight.
For FA cup, all teams are able to play even teams from other division.
