For a visa for Germany you will need to contact the German consultate directly. If in doubt you could even ask your travel agent or airline for advise and information on obtaining a visa, they should be able to tell you - if they can't they are doing a really bad job.
A visa is a specific document (which could just be a stamp in your passport) which allows you certain entry into a particular country. Eg. you would need a tourist visa for Germany and it would alllow you to enter, stay and visit Germany for a set period of time, and it would stipulate that you must leave Germany before the visa expires. There are different types of visas, eg. Student visas allow you to study in a country, working visas allow you to work etc. All visas would have set guidelines and expiry dates.
Your passport is your individual document to travel out of your own country and into others. When you enter and leave countries they will check your passport and may stamp it with the dates you entered or left. Visas may be included in your passport also, via stamps or special documents which are stapled inside.

A passport is your personal travel document that permits you travel around the world. You can not leave your country without one. A visa is an entry certificate into another country. Not all countries require these - only some. For example, as an Australian, we can travel to America for 90 days without a visa. However, if we travel to China, we need to obtain a visa before we can enter their country. The visa gets put into your passport either as a stamp or a sticker.

If you are an american going ot Germany for a month, you dont need a travel visa. If you are in ONE country for more than 90 days, then you will need a travel visa, work visa or student visa or whatever the purpose for being there that long is.
You're passport is your ID showing your nationality, and the visa is issued by the country you are visiting. The country wont let you in if you dont have a visa if it is required for that country.
Usually, if the US requires a Visa for that country, then we require a visa for that country.
Australia for example, they need a visa to get into America. So Australia said fine, you need a visa to get into Australia.

A passport is your identification. A visa is a stamp in your passport that says you have permission to travel into a specific country. For some countries, you cannot just enter at will. You would have to apply for entry. Once you get permission to enter the country from their government, the embassy would Stamp your passport. So, when you went to the immigration service of that country, they would see that you got permission to enter. The type of visa will tell the immigration officer what type of activities you are permitted to do. Hence, you have a "Working visa", or "Cultural activities Visa", "Missionary Visa" etc.

A visa is like a tax+permit you must pay and obtain before entering a country. Visa prices vary depending on: 1. Length of stay. 2.Whether your country and the country you are entering have agreements concerning visa policies, i.e.: Europe Union Members have easier visa policies for entering member countries.
