Usually, they don't know. First of all there is no 'the' groundhog. There are a number of local traditions involving groundhogs; some do not employ an actual living critter, while others usually involve a groundhog who is entirely cared for in captivity and only comes out in public once a year. Most commonly, one person or a committee will make a prediction based on whatever factors make sense to them, then bring out the groundhog on the appointed day and claim the critter communicated it to them. It isn't intended to be taken seriously.
The closest thing to an 'official' groundhog is Punxsutawney Phil, about whom you can learn more at www.groundhog.org

Well, in the split second between all the flash bulbs of the cameras that were casting the shadows of the trees, people, and other such things, wasn't casting shadows of trees, people, and other such things, the official "shadow looker" said that the groundhog didn't see it. They know that because the groundhog didn't pee in excitment!
