Would this normally happen to someone who is healthy if he has not been out on the bike in a while or is it something that should not be happening? What could this be?

This is something that should not be happening. I would advise him to go see a doctor as soon as possible. It is dangerous to be in freezing temperatures for an extended period of time. Hopefully he was dressed well and feels better soon. Good luck.

It could just be the cold weather. If he didn't have anything covering his nose/mouth, he probably breathed in too much cold air. The same thing happens to me if I'm outside for too long without something over my face. I would be concerned if he was elderly, not in good shape, or had a history of heart problems. If the pain lasts a long time (mine usually lasts about a half hour), see a doctor pronto!

Your friend was not smart to do this. 10 degree weather is not good for his lungs. Of course they will hurt...He froze the tissue. If he doesn't end up with an infection, he'll be lucky.
