Holy Name is the headquarters of the Chicago Archdiocese. Five Holy Martyrs on the South Side of Chicago is famous because Pope John Paul 2 did a mass there in the 70's.

Chicago's Catholic Churches are not much a tourist destination, but here are a couple of interesting spots:
Holy Name Cathedral on State Street - the cathedral of the Chicago Diocese.
Old Saint Pat's in the West Loop - site of the world's largest block party.
St. Michael's in Old Town - Beautiful inside.
St. Vincent DePaul Church in Lincoln Park (DePaul University)

I am sure there are others that may be significant, but not a lot that are real tourist destinations.

Holy Name Cathedral near Chicago Street and Rush St. There are older, more ornate type churches throughout the city, but I'm not familiar with them.
