I don't know if I would call this "really offensive", but my nickname in high school was bon-er. It actually started when I was in middle school. My shorts fell off when I got out of the pool. Even clear through high school, I never heard the end of it.

Some kid gave me the nickname "Rat" my freshman year of high school. He came up to me one day in the lunch room and flipped my hair up and was bugging me and he yells out "Look she has a bald spot shaped like a rat!!"

My dad called me Mop Head but it was affectionally meant.
I always used to say when I got a vehicle, I'd get customized plates reading MOPHEAD but then he started calling my little sister the same........

"Rosy" because when I was in 2nd grade I was really pale and my mom would dab blush on my cheeks for school. I didn't consider it offensive at all - that's about the only nickname I had.
