you can contact your local SS office...there is also a form that can but pulled up online...print it, fill it out and mail in required id's...they will send back your birth certificate..(or whatever form you sent in) along w/ your new card...

Go to the Social Security office... and bring your birth certificate along with other valid forms of ID. I tried it without the birth certificate and they wouldn't do it.

Look up your local SS office. Then take a day off from work because you have to wait in line and fill out paperwork and then wait in line and fill out more paperwork.
Make sure to ask what you need to bring before you go. you don't want to waste 2 vacation days at the SS office.
