Go to your county's web site. Then look under the court web link. There should be a link for family law documents. Look there for what you need. If you don't see it there, go to the circuit clerk's office and ask. Don't just find a blank, generic one on line (not specific to your circuit). Different states and circuits within the states have different requirements for petitions. Get one from your circuit. Good luck.

Whatever form custody takes, state laws require that the arrangements be in the best interests of the child.

In Phoenix, AZ, I found all the legal blank forms I needed at a stationary store next the the court house.
You may have the same success in your town.

most county's web pages have legal forms. you can also go to your yellow pages & look under the header "legal forms" and call around.
