If you are the victim, go to the police department. They can take your report and also give you help getting into a secure shelter where you will receive help for you and/or your children If there is domestic abuse in your home and you have children, if you do not report it, then you can go to JAIL if something happens to one of them.
To report a case of domestic violence not in your home, you must go to your local Dept. of Human Services Office and make a report. If you afraid of retaliation, keep in mind that DHS is not allowed to give out the name of the person reporting the incident.
If children are involved, please report this immediately.

Go to the police and fill out the form. Just go to the main desk, and tell them you'd like to report domestic abuse. They'll make sure they give you the right paperwork.

Call the police immediately. Don't know where you are, but if you are being abused in ANY way, you want to get out of there asap.

Good luck.
