Give him a birthday card and a gift card to his favorite store let him buy what he wants or needs. This is exactly what i told my wife to do at birthdays, christmas, and every other occassion.
Also, told her if she ever took me to another restaurant for my birthday and the staff came out and started singing that happy birthday song to me in a public place i'd divorce her so fast i'd make her head spin.
Men really don't get into that birthday gift giving crap like women do.

Get him a massage and a men's manicure package at a really nice spa. Men love massage but rarely get them. Massage packages are usually thought of as girly gifts. If you think he would be open to it I think it would be a perfect gift. You could also give him a massage at home. Make a path of rose petals lined with candles to a makeshift spa in your home...maybe your bedroom. Make sure there are lots of cozy, warm towels to surround his body. You could get a massage book and oil to learn some techniques. Afterwards, feed him a nice light meal. If he loves oysters...give those to him with some champagne or his drink of choice. Then let whatever happens happen. Is your guy too macho? Buy him a NICE watch. This is a status symbol for men.

I'm a girl so i think the best birthday gift would be of course depending on his age and job i'd say either a tie or a T-shirt would be something you could get him that would be put to good use!!

I go through same thing at my husbands birthday, what do you get a grown man that buys what he needs when he needs it!!
I've found nothing he likes more than lingerie.

You should get him a flat screen tv...then he could watch all his football games in high definition...lol...or a card with a gift certificate to his fav place
