Lack of support from teachers/parents/peers, failure to teach in a holistic way, some children are visual learners but are lectured at by the teacher, so they learn nothing. I had trouble with that a lot at high school where teachers would concentrate on one aspect such as visual learning and students who were better auditive or kinesthetic would miss out. The best kind of teaching is using all methods.
Teachers also need to be culturally sensitive and avoid being discriminatory.
Being bullied can distract a child from the work in school. Also having girlfriends/boyfriends. Learning disorders.
Difficulty, pressure and stress.
Test anxiety can cause a reasonably skilled student to make mistakes that could end up failing them.
Haven't had the role models to show that education is important.
